How about a decent sized gobstopper with a hole drilled through it so you can feed a string/shoelace through it and tie it... being a sweet, it will get smaller over time, but you'll be salivating and dribbling everywhere

There was a dominant girlfriend who caught me masturbating in her pantyhose. So as part of my punishment she stuffed the cum soaked pantyhose into my mouth (Cum side down on my tongue) Then stuffed 5 pair of her dirty pantyhose in after them and tied them in with the legs of the last ones. My mouth was so full I had chipmunk cheeks.

Thats my personal experience with unusual gags. I've seen bar soap and high heel shoes used as gags though. How about a nice big apple / unpeeled orange or a synthetic penis?

Look up "How to tie a monkey fist knot" The larger the diameter ball & rope, the bigger the knot.
For clarity, this has nothing in common with "fisting." It does have potential to be used as a gag though.

Another one which is kinda painful is clothespins on lips and tongue. Trust me it becomes very hard to talk!
