New hint: Do the difference first, the additions after.
PS: I added the 'T' because when I made this, I typed cutlery with double T by mistake

By the way I had a hard time googling for the word to be sure it is one word. Even wiki was a mix. That's the reason I edited my first post mentioning it is made of 2 words, I wasn't sure how people consider it. I just knew the fetish sense was one word.

Pascal's triangle was the thing I started with (although if someone didn't know about it, I think would assume a pyramid of numbers by counting squares).

I wanted to add something but I was afraid going into the ballance being too easy or too hard.
Simply 2 words, both are respecting the same rule, but not related to each other. 3 pics, 2 words.
I recently just loved to create puzzles, mostly because I love those in my games I play. I am trying different approaches but I can't make them complicated. Your's and Nina's were way above.

Actually I think the upside down idea is useless, I thought on something but it makes this more confusing.

Ok, now I'm more confused. I did have an idea that maybe this was something to do with eating, but the opposite which was the upside down thing but now that doesn't work.
I guess the chimp is a messy eater, but then I noticed he is holding the pineapple with his foot. Which could lead to FOOT FETISH?
The oyster is a shellfish, so somehow "not real" "shellfish" gets twisted into fetish?

The 2 words are 2 separate anagrams.
But I didn't show + or - signs to avoid making it too easy. Only if I did, the upside down image makes little sense.
Pineapple is one of the words.
Eating is not, but both images contain edibles (a good spot, not related to the answer but hints another word).
Also don't think strictly to the name of a fetish. But something related.
