Safewords applicable during punishment?

I play with no safeword, i know there is important risks, but that cnc is what excites me. I really set limits before play, so it is not “everything ok”, but i play no safeword, yes. It is something that do not start this way, require practice and need be really careful!

I'm genuinely curious here, and please be assured I am not doubting or being critical but how do you signal if something goes wrong?
Say for instance you experience chest pains, can't breath properly, develop a severe cramp or start to have a panic attack, what do you plan to do? Are you relying entirely on your dom being observant and hyper vigilant to what's happening?
Or is it more that you don't ever leave normal conversation mode, so you can always just say "Hey, my leg is cramping real bad we need to stop." or something like that?

Most part of it is virtual, but no risk less. But yes, i rely on dom
